• Arrival and Check In

    Plan to arrive 15 minutes before the time your doctor has requested, so you can park and not feel rushed. This also allows us more time to:

    • Answer your questions
    • Discuss any insurance concerns
    • Fill out registration paperwork

    What to Bring

    • Any medical devices you need, such as glasses or dentures
    • Completed forms, such as advance directive, living will, medical or durable power of attorney, or health care proxy
    • Insurance card
    • Photo identification (such as a driver’s license, passport or ID card)
    • List of medications you’re taking, including any vitamins or supplements (not the actual medications)

    As a precaution, leave any valuables, such as jewelry, cash, computers or credit cards, at home.
